1. Acceptance of Terms
By using Wild Ridge Nutrition telehealth services, you agree to these Terms & Conditions. If you do not agree, you may not use our services.
2. Eligibility
You must be at least 18 years old or have parental/guardian consent to use our services. Telehealth services may not be suitable for certain medical conditions or emergencies.
3. Telehealth Services
Sessions may be conducted via phone or PracticeBetter.
We do not guarantee specific results from treatment.
Telehealth is not a substitute for emergency care. If you experience a medical emergency, call 911 or go to your nearest emergency facility.
4. Patient Responsibilities
You agree to:
Provide accurate and complete information about your health.
Follow instructions given during sessions.
Ensure you are in a private, secure location during telehealth sessions.
5. Payment, Refunds & Insurance
Payment is due at the time of service unless otherwise arranged.
We do not accept insurance directly but can provide a superbill for reimbursement.
Refunds are determined on a case-by-case basis. Requests should be submitted in writing, and any approved refund will be issued at the sole discretion of Wild Ridge Nutrition.
6. Cancellation & Rescheduling
Cancellations within 24-hours of the appointment or no-shows incur a $50 fee.
Arriving more than 15 minutes late constitutes a no-show, incurring a fee and require rescheduling.
Appointments can be rescheduled at any time prior to the 24-hour window.
Prepaid services or packages have expiration dates; unused services after expiration are forfeited.
By booking, clients acknowledge and accept this policy.
7. Confidentiality
We maintain confidentiality as required by HIPAA. You acknowledge that telehealth sessions may have risks related to electronic communication.
8. Limitation of Liability
Wild Ridge Nutrition is not liable for:
Technical failures beyond our control.
Any indirect, incidental, or consequential damages from use of our services.
Outcomes from treatment, as results vary by individual.
9. Termination
We may terminate or suspend services if:
You violate these Terms.
You do not comply with treatment plans or payment policies.
10. Changes to Terms & Policies
We may update these Terms. Changes will be effective upon posting to our website. You are responsible for reviewing updates.
11. Governing Law
These Terms are governed by the laws of West Virginia.
